Change of Address
You must let us know straight away (in writing if possible) if you change your name or address. This is to make sure that you can continue to be registered with us, and to keep our records up to date.
If you move to an address which is outside our Practice Boundary, you will have to register with a new Doctor. This is because we cannot visit or treat patients who live outside this boundary.
